Home Health Care furnishes an efficient and cost effective alternative to hospitalization or other institutional care. Our services are provided by a team of highly trained home care professionals dedicated to going above and beyond to ensure the care and comfort of patients and their families, while simultaneously addressing individual needs and providing the highest quality, patient-focused, and loving care available anywhere.

Additionally, the services we offer are not provided exclusively to seniors. Whether you’re a new mom, an injured athlete, or anyone recovering from an operation or illness, LifeSpring In-Home Care Network pledges to compassionately care and support those individuals in the comfort of their own homes. Services are provided by the following Home Care Team:

RN Case Manager
This licensed and trained professional coordinates and manages the patient’s care, evaluates their condition and needs, and implements the Physician’s treatment plan.

Skilled Nurse
Assists the RN Case Manager in carrying out the treatment plan, providing care, and monitoring the condition of the patient.

Rehabilitation
Including Physical, Occupational, and Speech therapists who work closely with care givers and patients to achieve a higher level of independence.

Medical Social Worker
Assists patients and families with accessing financial resources, facilitates emotional and community support and counseling, and transitioning to a new environment such as assisted living or a move into a relative’s home.

Certified Home Health Aides
Assist the patient with bathing, grooming, ambulation and the activities of daily living.

 

F.A.Q.

Who Pays for Home Health Care?
Home Health Care is a benefit under Medicare Part A with no co-pay or deductible. Most insurance companies pay for home health care at 80 – 100% depending on circumstances. Medicaid and other programs pay for home health care under certain circumstances. LifeSpring In-Home Care Network will verify and authorize insurance and other benefits on behalf of our patients.
