Easy Referral Process
A simple phone call is often all you need to make a referral, followed by a fax or email to provide pertinent patient information.

Timely Response
Patients are evaluated within 24-48 hours of referral.

RN Case Manager: This licensed and trained professional coordinates and manages the patient’s care, evaluates their condition and needs, and implements the Physician’s treatment plan.

Skilled Nurse: Assists the RN Case Manager in carrying out the treatment plan, providing care, education, and monitoring the condition of the patient.

Rehabilitation: Including Physical, Occupational, and Speech therapists who work closely with caregivers and patients to achieve a higher level of independence.

Medical Social Worker: Assists patients and families with accessing financial resources, facilitates emotional and community support, provides counseling, and helps with transitioning to a new environment such as assisted living or a move into a relative’s home.

Certified Home Health Aide: Assists the patient with bathing, grooming, ambulation and the activities of daily living.

About Us

We are a family owned, locally operated and nationally supported home care agency. We provide exclusive private duty services for individuals or families in Alameda and Contra Costa Counties who need assistance with daily living. Whether it’s one hour a day or all day, every day, our services are specifically tailored to meet the client’s needs by providing Companion and Homemaker, Certified Home Health Aide, Live-In Care and Companionship, Facility Sitter Service anywhere in the Alameda and Contra Costa Counties. While the clients we serve are mostly in Alameda and Contra Counties, we do not mind extra mile for services like doctor's appointment, events and errands. Because some clients may require less extensive help, we also offer Housekeeping and Chores, Handyman, Errand Runs, “Bed and Breakfast”.

We realize the caregivers’ first and foremost function is to help restore the dignity of the ones they care for with utmost compassion and skills. We strive to put the care in the caregiving.
